The order in which AI names medical clinics in Brazil is decided by a directory.
We measured the Brazilian clinic niche. A single domain appeared on most of the themes we measured, and it is the one publishing the "20 most recommended" lists. No clinic repeated. The good news is in the other half of the measurement.
The size and the state of the niche

One directory and sixteen clinics from different cities
We measured the page-one positions for this niche’s real buying questions. Clinics hold most of the space, but each appears exactly once, on its own question and in its own city. The only domain that repeats is a medical directory.

Two readings, both useful. First: the directory is the only repeat, and it is the one publishing the "20 most recommended"meaning the order in which AI names clinics was set by a third party. Second: the clinics that appeared are from Belém, Goiânia, Recife, Ribeirão Preto, Brasília, Rio and Curitiba. None is national. This is won city by city.
And technical preparation is halfway there
We tested the sites of five of the country’s largest health institutions. Two passed both basic automated-reading tests; the other three passed only one. None was shut out entirely, and none is complete.
It is a picture of a market in transition: the sites exist and are reachable, but most of them still do not describe themselves in a way a machine can repeat accurately. For a smaller clinic that is good news. The bar has not gone up yet.
And the bar that matters here is local. A national hospital does not contest someone looking for physiotherapy in their own neighbourhood. What contests it is the clinic that wrote down clearly what it does and where it is.
What this means in practice
The "most recommended" list is not yours
The directory builds the order on its own criteria, review count, subscription tier, time listed. When an AI assistant summarizes that page, it repeats the order without explaining where it came from.
The fight is per city, not per country
Every clinic in the measurement appeared in its own city’s context. That shrinks the opponent: you are not competing with a national network, you are competing with the clinics in your neighbourhood that described what they do better.

Questions from people who run clinics
- Health advertising is tightly regulated. Is that a problem?
- We do not advertise procedures, promise results, use before-and-after imagery or make any clinical claim. The work is institutional information: who the clinic is, where it is, which specialties it covers, hours, staff, what people say about the service. None of that replaces or loosens your professional council’s rules, validating what may be published stays with the clinic’s technical officer.
- We are already listed in the directories. Is that not enough?
- Listing puts you in the list; it does not choose your position or what is said about you. And it is a page you do not control: the entry can be stale, the hours wrong, a specialty missing. The measurement shows the directory appearing on most of the measured themes, so it is worth checking what it says about you before anything else.
- Our patients come by referral. Does this change anything?
- Referral is still the main door. What changed is the next step: someone given a referral checks the name before booking, and increasingly checks by asking an assistant. If the answer is vague, stale, or about another clinic with a similar name, you lose an appointment that was already yours.
- We are a small clinic in a single city.
- That is exactly the profile that appeared in the measurement. Every clinic that took a page-one position is regional, each on its own city’s question. You do not need to be big, you need to be the clearest in your region.
